No. We are not an aged care provider ourselves. We receive referrals from aged care providers and work closely within that network to support older Australians with physiotherapy and allied health services.

Referrals can be submitted online via our referral form: 365 Rehab Referral Form.
If you are a new organisation we have not partnered with before, we prefer to arrange an introductory meeting (either face-to-face or via Zoom or Teams) to establish connection and ensure expectations are clearly aligned.

We currently provide mobile physiotherapy and aged care support in:

St George, Mt Druitt & Blacktown, The Hills District, The Shire & Parramatta.

We are working closely with established networks to expand our aged care offerings. These services are scheduled to commence in early 2026:

– Domestic Assistance Division – practical home support including cleaning, laundry, meal preparation, and shopping.

– Telehealth Psychotherapy – secure digital therapy sessions to support mental health, resilience, and quality of life for older adults.

We accept referrals for community physiotherapy. Government funding schemes such as HCP or CHSP are managed directly by aged care providers. Families may also request our services privately, and where needed, we can refer back to our trusted aged care provider partners for more information about funding options and eligibility.
